Anche quest’anno, grazie alla legge 244/07 (finanziaria 2008), con la dichiarazione dei redditi 2012 potete continuare a destinare il 5 per mille della vostra IRPEF, senza che questo comporti alcun aumento della vostra tassazione, a favore della Fondazione Livio Sciutto-O.N.L.US. La Fondazione è regolarmente iscritta all'Anagrafe Unica delle O.N.L.U.S. - protocollo n. IPSO712100001. Ecco perché potete fare un gesto che a voi non costa nulla ma che per noi è di vitale importanza.

COME FARE

Il 5 per mille - che non sostituisce in alcun modo l’otto per mille previsto per lo Stato, la Chiesa Cattolica e altre confessioni religiose – per essere destinato alla Fondazione ha bisogno della vostra firma.

I modelli per la dichiarazione dei redditi 2007 (CUD 2008, 730/1 bis e UNICO hanno uno spazio dedicato al 5 per mille. Lì troverete quattro caselle tra cui scegliere. Per far sì che il vostro 5 per mille venga destinato alla Fondazione Livio Sciutto O.N.L.U.S. seguite le seguenti istruzioni:

Firmate all’interno del riquadro “organizzazioni non lucrative di utilità sociale di cui all’articolo 10 del decreto legislativo 4 dicembre 1997, n. 460 e successive modificazioni e integrazioni”;
Inserite il nostro codice fiscale 90048330097 nello spazio sottostante come raffigurato nell’esempio qui sotto.

Se esonerati dall’obbligo di presentazione della dichiarazione, ma con redditi soggetti a IRPEF, potete compilare la scheda per la scelta della destinazione dell’8 per mille e del 5 per mille dell’IRPEF. La scheda va presentata, in busta chiusa, entro il 31 luglio 2008, con le seguenti modalità:

Allo sportello di un ufficio postale che provvederà a trasmetterle all’Amministrazione finanziaria. Il servizio di ricezione della scheda da parte degli uffici postali è gratuito;
Ad un intermediario abilitato alla trasmissione telematica (professionista, CAF, ecc.). Quest’ultimo deve rilasciare, anche se non richiesta, una ricevuta attestante l’impegno a trasmettere le scelte. Gli intermediari hanno facoltà di accettare la scheda e possono chiedere un corrispettivo per l’effettuazione del servizio prestato.

La busta da utilizzare per la presentazione della scheda deve recare l’indicazione “SCELTA PER LA DESTINAZIONE DELL’OTTO E DEL CINQUE PER MILLE DELL’IRPEF”, il codice fiscale, il cognome e nome del contribuente.

The Livio Sciutto Foundation for Medical Research, a Non-profit Social Organisation, was established in October 2007, the deed of incorporation drafted by Notary Public Riccardo RIDELLA, from Genoa. We were born with a small budget but with the spirit to carry out great projects to fight disease and improve quality of life for people.

The Livio Sciutto Foundation has received, as donation, the patrimony and historical-scientific archives of the Scienza e Vita (Science and Life) Foundation, organisation founded by Prof. Lorenzo Spotorno in 1994 and which ceased its operations on 1st January 2008.

The Foundation exclusively pursues goals of social solidarity by promoting the development of human knowledge in the medical-scientific domain, particularly orthopaedia, and supporting the prevention and care of all human pathologies through clinical research and the clinical application of the results obtained.

The Foundation shall develop its research goals in the following sectors:

Mini-invasive surgical techniques, including the development of new, more conservative prostheses;
Computer-assisted surgery, both for hip and knee procedures;
Development of stem cell cultures for bone and tissue regeneration;
Technologically innovative materials for orthopaedic prostheses (post-marketing study);
Surgical methods integrated with anaesthesiology and physiotherapeutic aspects that would allow shorter hospitalisation times, fast functional recovery and less pain;
Professional update methodologies for Orthopaedic physicians;
New diagnostic-therapeutic procedures in cooperation with the Radiology Department;
Design and update of software to improve services and social-sanitary interventions for the prevention and care of all human pathologies.

This activity of clinical research is carried out thanks to the significant collaboration of the physicians and department of prosthetic surgery of the Santa Corona Hospital in Pietra Ligure, as well as collaboration agreements with other entities, institutions, individuals and health care structures.

The Foundation participates with its own researchers in national and international congresses; it coordinates visits by surgeons from all over the world for live update sessions in prosthetic surgery; participates in 'live' sirgery courses in Italy, Germany, Switzerland, Sweden, Venezuela, Argentina, Libya, Iran and India; and, it arranges the publishing of articles in the most important orthopaedia magazines.

The Foundation possesses an IT system that allows the surgeons who weekly gather in the special convention room a direct view of the procedures being performed in the prosthetic surgery room of the Santa Corona Hospital, as well as digitalised images of X-ray exams. Furthermore, on occasion of national and international conventions/congresses, video editing, case studies and statistical studies are produced, as well as publications, densitometry touch-ups of X-ray scans, and multimedia presentations.
